Job summary

DaVita is hiring an RN Case Manager in Pasadena, CA, a hybrid role combining clinical oversight with care coordination for ESRD and CKD patients.

The position requires ADN with RN licensure, five years of clinical nursing experience, and three years in renal care or equivalent case-management duties. Capella and DaVita software familiarity is expected; CPR certification is required.

Qualifications

• ADN required; BSN preferred (three-year diploma may substitute).
• Current RN license in state of practice.
• Continuing education credits maintained as required by state.
• Minimum of five years’ clinical nursing experience.
• Minimum of three years’ renal nursing experience preferred.
• Case-management experience or equivalent: assess needs, develop care plans, coordinate services across teams.
• CNN or CCM preferred.
• Knowledge of data/clinical outcomes management.
• Experience as a change agent and CQI techniques.
• Healthcare performance coaching experience required.
• CPR certification current.
• Capella implementation familiarity.

RN Case Manager

Location: Pasadena, CA (Hybrid)

Schedule: Monday – Friday (flexible)

Employment Type: Full‑time

Coordinates and facilitates quality, cost‑effective care for patients with ESRD and CKD, collaborating with the healthcare team, patients, families, providers, and community resources to minimize fragmentation of care.

Qualifications

• A.D.N. degree from an accredited nursing school required; B.S.N. preferred; a three‑year accredited diploma program may substitute for a nursing degree.
• Current registered nurse (RN) license in the state of practice required.
• Continuing education credits maintained as required by state of practice.
• Minimum of five (5) years’ experience in clinical nursing.
• Minimum of three (3) years’ experience in renal nursing preferred.
• Case‑management experience required; if not, a minimum of three (3) years of experience assessing patient needs, developing and managing individualized care plans, coordinating services across interdisciplinary teams, and supporting patients through complex transitions.
• Certified Nephrology Nurse (CNN) or Certified Case Manager (CCM) preferred.
• Demonstrated knowledge and understanding of data and the ability to manage clinical, financial, and patient‑satisfaction outcomes.
• Demonstrated experience and effectiveness in a change‑agent role.
• Demonstrated knowledge and understanding of continuous quality improvement (CQI) techniques.
• Previous experience in healthcare performance coaching required.
• Current CPR certification required.
• Ability to modify personal practice patterns to adapt to new electronic processes and increased productivity expectations related to Capella implementation.
• Current driver’s license in the state of employment with a positive driving record and ability to meet the employer’s insurance coverage requirements.
• Basic computer skills and proficiency in Microsoft Word and Outlook required.
• Functional proficiency with DaVita clinical software programs, including Capella, required within 90 days of employment.
• Home office with internet connectivity of at least 1 MB upload and 1 MB download required.

Wage Range: $41.00 – $55.00 per hour.
