Job summary

Excellus BCBS is seeking a Case Management professional to conduct program activities in line with CMSA/NCQA standards and NYS DOH requirements. You will identify members, assess needs, educate, coach, and coordinate care with providers and community resources.

Join a cross-functional team to implement member-centric interventions, develop comprehensive care plans, and optimize health outcomes while managing cost drivers.

Qualifications

• Associate degree required; Bachelor’s degree preferred.
• Active NYS RN or Registered Dietitian or Physical Therapist licensure, or LMSW; or LCSW; or LMHC required.
• Minimum of three years of clinical experience; Case Management experience preferred.
• Proficiency with Microsoft Office Suite.

• Manage case management activities in accordance with CMSA/NCQA standards.
• Collaborate with providers and community resources to develop care plans.
• Coordinate interdisciplinary care and education for members.
• Maintain confidentiality and comply with regulatory requirements.
