Job summary

The Mount Sinai Morningside Case Management team is seeking an experienced RN Case Manager for night shift to coordinate care for inpatients in the ED and hospital units. You will assess admission necessity, plan and monitor the care trajectory, collaborate with physicians, social workers, and home care services, and drive discharge planning to optimize LOS and outcomes.

Requires NYS RN license; CCM and PRI certifications preferred; Bachelor’s degree in nursing with 3–5 years acute care

Qualifications

• Bachelor’s degree in nursing required; Master’s preferred.
• Current NYS RN license required.
• CCM certification preferred; re-credentialing expected.
• PRI Certification (to be obtained within 1 year).

Responsibilities

• Reviews all new admissions to identify utilization review, discharge planning, and case management needs.
• Performs continued stay reviews to justify inpatient status.
• Collaborates with physicians and other clinicians to expedite testing, treatment and consultations.
• Documents UR notes in the medical record and participates in discharge planning.
• Identifies and reports reasons for readmissions and works on strategies with the team.
