Qualifications

• RN license required and active.
• Nursing degree; bachelor’s preferred.
• Experience in hospital case management or utilization review is desirable.

Responsibilities

• Coordinate care for a caseload with physicians and nurses.
• Perform discharge planning and utilization reviews.
• Provide assessment, planning, facilitation and advocacy for services.
