Responsibilities

• Coordinate discharge planning for an assigned caseload with physicians, nursing staff and other health professionals.
• Collaborate with patient/family and interdisciplinary team to ensure quality, efficient care and appropriate resource use.
• Complete admission, continued stay and discharge reviews per regulations and guidelines.
