• Perform discharge planning to ensure efficient utilization and quality outcomes.
• Perform admission, continued stay, and discharge reviews per regulations and Utilization Management Plan.
• Collaborate with patient/family and interdisciplinary team, including payers.
• Document and ensure compliance with organizational policies and utilization management procedures.
