Job summary

ChristianaCare Hospital in Newark, DE seeks an RN Case Manager with discharge planning experience in an acute care setting. This Day Shift role (Mon-Fri, 8:00 a.m.–4:30 p.m.) includes no holidays or weekends and features call rotation every seven weeks.

You will coordinate care, drive patient progression, and lead discharge planning as part of an interdisciplinary team to establish timely, appropriate transitions across the care continuum.

Qualifications

• Active RN license in DE or Compact State.
• BSN required.
• A minimum of three years of experience as an RN in an acute care hospital setting is required.
• Critical Care RN with a minimum of three years of experience are encouraged to apply.

Responsibilities

• Identify patients with post-acute care, placement, and discharge planning needs based on comprehensive assessment.
• Establish and implement discharge plans in collaboration with physicians, nurses, and other care providers.
• Review admission assessments and ensure multidisciplinary care plans meet patient needs and outcomes.
• Identify and help remove barriers to care to facilitate patient progression.
