Job summary

Christiana Care Health System in Newark, DE is seeking an RN Case Manager with discharge planning experience in an acute care hospital setting. Hours are Mon-Fri 8a-4:30p with call rotation every 7 weeks; no holidays or weekends.

The role coordinates care, collaborates with the interdisciplinary team, and develops discharge plans to ensure timely transitions. External candidates are eligible for a 10K sign-on bonus.

Qualifications

• Active RN license in DE or a Compact State.
• BSN is required.
• 3–5 years of acute care hospital experience.
• In-patient case management experience preferred.
• BLS certification preferred.
• CMC must be obtained within 18 months of eligibility.

Responsibilities

• Identify patients with post-acute care and complex discharge planning needs.
• Anticipate and establish a discharge plan with the care team.
• Review admissions and ensure a multidisciplinary care plan.
• Identify system barriers to care and help remove them.
