Job summary

Logan Health Whitefish seeks an RN Case Manager (Discharge Planner) to provide seamless discharge planning and coordination. The role requires current Montana RN license and at least 2 years of acute or skilled nursing experience, with preference for care management experience and certification readiness.

Qualifications

• Current Montana RN License or multi-state compact license to practice in Montana.
• Minimum of two (2) years’ experience in acute or skilled nursing care.
• Experience with care management processes, including discharge planning, insurance reviews, and hospice issues preferred.
• RQI Provider (BLS) within 7 days of hire/transfer.
• ALS (ACLS) within 6 months of hire/transfer.
• PALS within 6 months of hire/transfer.

Responsibilities

• Assess patient needs and plan care to achieve defined outcomes.
• Guide patients and families through information and decisions within the health care system.
• Identify appropriate patients for care management and discharge planning based on needs and risk factors.
• Develop a network of services and coordinate care across the continuum.
• Establish and maintain methods for tracking patient progress and coordinate resources with community services.
• Participate in quality improvement and ensure confidentiality in all patient interactions.
