Job summary

Baxter Health in Mountain Home, AR is seeking an experienced RN to join our Continuity of Care team as Case Manager - Discharge Planner. You will develop discharge plans, recommend levels of care, and coordinate with the medical team to ensure smooth transitions.

The role requires nursing licensure in Arkansas or a compact state, a nursing degree, and strong communication and computer skills; experience in utilization review or case management and CCM/ACM certification are preferred.

Qualifications

• RN licensure in Arkansas or compact state in good standing.
• Graduate of an accredited nursing program.
• Experience in Utilization Review or Case Management preferred.
• ACM or CCM certification preferred.
• Bachelor's Degree preferred.

Responsibilities

• Develop discharge plans and recommend levels of care.
• Engage in patient assessment and collaboration with the healthcare team.
• Monitor patient progress and modify discharge plans as needed.
• Coordinate information with post-hospital vendors to support recovery.
• Ensure medical necessity is continually assessed.
