Job summary

Qualifications

• Current state RN license is required.
• Associate's degree required; BSN preferred.
• BLS certification required; MCG experience preferred.

Responsibilities

• Leads and coordinates discharge planning, including patient transfers and referrals.
• Monitors patient and family satisfaction; respond to questions and complaints.
• Documents case management activities and outcomes.
• Participates in performance improvement and continuing education.
• Supports department operations and may be on-call.
