Job summary

Doctors Hospital of Laredo, in Laredo, TX, is a 183-bed acute care facility that offers a range of medical services. A professional registered nurse who plans, manages, coordinates, implements, and evaluates the ongoing care and psychosocial needs of the patient.

The role includes monitoring patient outcomes, coordinating with the health care team, and on-call discharge planning after 5 pm and on weekends, contributing to quality, cost-effective, medically appropriate services.

Qualifications

• Current RN License in the state of Texas.
• Two (2) years clinical experience in hospital setting.
• BLS required.
• Prefer Utilization Review/Discharge Planning background.

Responsibilities

• The case manager is responsible for ongoing monitoring and evaluation of patient outcomes and for the provision of quality, cost- effective and medically appropriate services.
• Collaborates and assist members of the health care team, the patient and the family to determine home needs for effective, efficient, and appropriate discharge care.
• On-call rotation for discharge planning after 5 pm and on weekends.
