• Current RN License in Texas.
• Two years clinical experience in hospital setting.
• BLS required.
• Prefer Utilization Review/Discharge Planning background

Responsibilities

• The case manager is responsible for ongoing monitoring and evaluation of patient outcomes and for the provision of quality, cost-effective and medically appropriate services.
• Collaborates and assist members of the health care team, the patient and the family to determine home needs for effective, efficient, and appropriate discharge care.
• On-call rotation for discharge planning after 5 pm and on weekends
