Job summary

The Judge Group is seeking an experienced Registered Nurse Case Manager to join an acute hospital team in Bangor, ME. The right candidate enjoys direct patient interaction, coordinating care across disciplines, and ensuring safe and timely discharges.

Responsibilities include admission and ongoing case management, collaborating with physicians and social workers, and facilitating discharge planning. Active RN license and Cerner experience are required, with strong communication and teamwork

Qualifications

• Active and unrestricted RN license.
• Experience in acute hospital case management.
• Strong bedside nursing experience.
• Experience with Cerner EHR.
• Excellent communication and care coordination skills.
• Ability to collaborate with multidisciplinary teams.

Responsibilities

• Conduct admission and ongoing case management assessments to evaluate medical necessity, level of care, and progression of treatment.
• Review patient cases to ensure appropriate utilization of hospital resources and regulatory compliance.
• Collaborate daily with physicians, nurses, social workers, and other team members to develop patient care plans.
• Identify and address barriers to discharge and help patients transition to the next level of care.
• Coordinate post-acute services and community resources for continuity of care.
• Facilitate proactive discharge planning to optimize length of stay and reduce delays.
• Complete CMS documentation including IMM and MOON forms.
• Contribute to Readmission reduction and patient outcome improvements.
