Job summary

Winchester Hospital, part of Beth Israel Lahey Health, seeks a Case Manager to support our Magnet-designated 183-bed facility. You will help plan and coordinate patient care and discharge with patients, families, and the care team.

Minimum BSN or enrollment within 5 years and MA RN license are required; prior case management experience is preferred. You will work with the multidisciplinary team to ensure safe, high-quality outcomes for patients.

Qualifications

• BSN or enrolled within 5 years of hire date.
• Massachusetts RN license required.
• Previous case management experience preferred.

Responsibilities

• Utilize case management to plan and coordinate discharge for patients.
• Build therapeutic relationships with patients, families and care partners.
• Collaborate with the multidisciplinary team to optimize outcomes.
• Assess daily workflow, census, admissions, transfers and discharges.
• Coordinate next level of care with patients, families and insurers.
