Job summary

Dignity Health - Marian Regional Medical Center in Santa Maria, CA is seeking a Case Manager RN to coordinate care across the continuum of illness, promote effective utilization, and lead the interdisciplinary team to achieve optimal outcomes.

You will perform admission assessments, develop discharge plans, evaluate patient needs, and collaborate with physicians, social workers, and other staff to ensure timely, safe, and patient-centered care.

Qualifications

• High School Diploma or GED, upon hire.
• Graduate of an accredited registered nursing program, upon hire.
• Registered Nurse: CA, upon hire.
• Basic Life Support - CPR BLS, upon hire.
• Driver's License - CA, upon hire.
• Auto Insurance, upon hire.

Responsibilities

• Utilizes the nursing process to provide safe physician ordered care for patients.
• Assessment- assesses the patient condition at each visit noting significant changes.
• Planning- develops a patient specific plan of care in collaboration with the patient and physician.
• Implementation- Oversees or carries out the treatment plan with infection control.
• Evaluation- Regularly evaluates patient needs and progress toward goals.
• Assists with coordination of patient-care activities and ensures interdisciplinary collaboration.
