Qualifications

• Bachelor's Degree in Nursing is required.
• Minimum of two years of healthcare experience.
• One year of case management experience.
• Strong written and oral communication, diplomacy and analytical skills.
• Self-directed, assertive problem-solving and organization in a high-volume setting.
• Basic computer skills and proficiency with an electronic patient record.

Responsibilities

• Act as an active member of the interdisciplinary team and contribute to goal setting.
• Consult with clinical teams and departments; participate in program development and quality improvement initiatives.
• Provide age-appropriate assessments, data interpretation, and delivery of interventions.
• Coordinate discharge planning for assigned caseload and facilitate appropriate lengths of stay.
