Job summary

The RN Case Manager role at Cynet Health is a 13-week contract based in Illinois. You will oversee discharge planning and care coordination for an assigned caseload of 13-25 patients in an acute hospital setting.

Strong clinical judgment and collaboration with a multidisciplinary team are essential. You will conduct utilization review, develop individualized care plans from admission to discharge, manage throughput and length of stay, and participate in family meetings and care coordination

Qualifications

• Minimum of 2 years experience in case management.
• Registered Nurse license in Illinois.
• BLS (AHA) certification.

Responsibilities

• Manage caseload of 13-25 patients.
• Focus on utilization review and discharge planning.
• Develop plan of care from admission through discharge.
• Manage throughput and length of stay to optimize resources.
• Interdisciplinary rounding and care coordination.
• Family meetings, insurance authorization, and discharge planning.
• referrals for DME, home health, or outpatient services.
• Team conferences Monday through Friday for rehabilitation.
