Job summary

Memorial Medical Center in Las Cruces, NM is seeking a registered nurse to lead discharge planning, coordinating with payers, programs, and community services to ensure a safe transition from hospital to appropriate care settings.

You will work with a team across long-term care, rehab, home care, and social services, supporting advanced directives and end-of-life planning to improve continuity of care for patients and families.

Qualifications

• Current RN license in the state of practice.
• Associate's degree from an accredited nursing program.
• Certification by the National Association of Healthcare Professionals or InterQual is desirable.
• CCM or ACM certification desirable.

Responsibilities

• Assist with discharge planning and coordinate care resources for patients.
• Coordinate with third-party payers, government programs and community services.
• Coordinate placement and support services across long‑term care, rehabilitation, home care and related services.
• Support end‑of‑life planning and advance directives in collaboration with social services.
