Qualifications

• Active RN license in Florida or a compact state.
• 2-4 years of acute care case management experience.
• InterQual proficiency and discharge planning experience.

Responsibilities

• Conduct initial assessments and discharge planning for patients.
• Collaborate with multidisciplinary team to develop and implement care plans.
• Maintain effective communication with healthcare teams and patients to ensure continuity of care.
• Utilize Meditech and other documentation systems for patient records.
