Qualifications

2 years experience in a clinical setting (e.g. home health, inpatient, physician office, clinic).
RN licensure in Oklahoma or multi-state via eNLC.
BLS certification within 30 days of hire.

Responsibilities

Complete a comprehensive assessment of patients' clinical, psychological and financial needs using available resources.
Coordinate timely transfers to appropriate levels of care based on clinical needs and utilization criteria.
Develop, implement and revise discharge plans, including referrals to health care and community organizations.
Communicate discharge care plan and changes to patient, family and healthcare professionals.
Assist physicians and hospital staff with optimal utilization of resources and timely discharge planning.
Coordinate services between hospital departments to facilitate timely patient discharge.
