Job summary

Longleaf Hospice LLC is seeking an RN Case Manager in Georgia to provide nursing care to hospice patients in the community. You will coordinate care with the interdisciplinary team, assess patient needs, and support families with education and guidance.

The role requires a current Georgia RN license, CPR certification, and 1–2 years of home care experience. A valid driver’s license and reliable vehicle are essential for home visits.

Qualifications

• Good verbal and written communication and problem solving skills.
• Responsible, valid driver’s license; reliable vehicle for home visits required.

Responsibilities

• Completes an initial assessment of patient and family to determine hospice care needs; provides a complete physical assessment and history of current and previous illness
• Uses health assessment data to determine nursing diagnosis
• Triages calls and assists patient/primary caregiver with problem resolution
• Teaches and counsels patients and families in meeting nursing and related needs
• Makes visits for symptom management and support as needed
• Modifies patient treatment plans as indicated by patients' responses and conditions
• Maintains current, accurate, detailed reports in accordance with policy
• Monitors, records and reports symptoms and changes in patients' condition to the IDG
• Provides health care instructions to the patient as appropriate per assessment and plan
• Obtains and receives physician orders as required
• Pronouncement of patients

The RN Case Manager is responsible for providing nursing care to patients, including identifying and coordinating patient/family care with other members of the Interdisciplinary Team and health care providers.

Requirements

• At least one or two years of recent home care experience; hospice experience preferred

• Responsible, valid driver’s license; reliable, insured, automobile for making home visits required
