Job summary

Vibra Hospital of Sacramento in Folsom, CA is seeking a Case Manager to coordinate clinically complex patient care across the continuum, ensuring quality outcomes and effective discharge planning. A licensed professional with hospital case management experience will facilitate services and navigate resources for patients and families.

Required licensure (RN/OT/PT/SLP or MSW) with a minimum of 3 years in hospital case management and a Vibra-approved BLS certification.

Qualifications

• Current, valid license to practice as a Registered Nurse, Occupational Therapist, Physical Therapist, Speech Therapist or Licensed Social Worker in the state of employment required.
• Master’s degree in Social Work and at least three (3) years of hospital case management experience required.
• Current BLS certification from a Vibra-approved vendor required.

Responsibilities

• Coordinate clinically complex patient’s care across a continuum; ensure and facilitate the achievement of quality clinical and cost outcomes.
• Negotiate, procure and coordinate appropriate services and resources needed by the patients; intervene to address issues/concerns.
• Conduct comprehensive discharge planning and ensure appropriate sequencing of treatment goals.
