Job summary

Vibra Hospital of Central Dakotas in Mandan, ND is seeking a Case Manager to coordinate care for clinically complex patients and ensure successful discharge planning. The role involves navigating services, collaborating with care teams, and maintaining high standards of quality and cost efficiency.

Qualified candidates will have an active RN, MSW, or LSW license in the state, plus Vibra-approved BLS certification.

Qualifications

• Current, valid license to practice as a Registered Nurse, MSW or Licensed Social Worker in the state of employment required.
• Current BLS certification from a Vibra-approved vendor required.

Responsibilities

• Coordinate clinically complex patient care across a continuum to achieve quality clinical and cost outcomes.
• Negotiate, procure and coordinate services and resources needed by patients.
• Contribute to development and execution of a comprehensive discharge plan; ensure proper sequencing of treatment goals.
