Aveanna Healthcare in San Bernardino, CA is seeking an RN Case Manager - Clinical Supervisor to lead our clinical team, mentor field staff, and ensure high-quality home health care. The role offers a hybrid work model with a blend of office and patient-home visits, autonomy in scheduling, and growth opportunities within a nationwide organization.

Compensation includes an $85,000 yearly salary with quarterly incentive potential, cellphone and mileage reimbursement, plus a $5,000 sign-on bonus.

Qualifications

• Must hold an RN license in the state where practicing.
• Minimum 2 years of RN nursing experience within the last 5 years.
• Willing to travel to patients' homes in designated territory; reliable transportation and valid driver's license.
• Current CPR certification from AHA.
• Willing to supervise nurses providing in-home care to all ages.
• Opportunities to advance professionally.

Responsibilities

• Promote and enhance clinical staff expertise to ensure safe, high-quality nursing care.
• Review orders, update plans of care and documentation as needed.
• Assist with interviewing, training and validation of new clinical staff.
• Coordinate start of care assessments and re-evaluations when releasing patients home.
• Collaborate with operations and business development teams.
• Oversee internal case management and development of nursing talent.
• Ensure adherence to policies and regulatory requirements.
• Ensure quality assurance of clinical documentation.
