RN Case Manager - Clinical Supervisor

Work Location: Combination of Aveanna Palmdale office and patients' homes in the Palmdale area

Compensation: $80,000 to $85,000/year salary plus incentive potential, cellphone and mileage reimbursement

Responsibilities of the RN Case Manager:

• Promote and enhance the clinical expertise of staff to deliver safe and quality nursing care
• Review physicians’ orders, update care plans and documentation as necessary
• Assist with interviewing, selecting, training, and validating new clinical staff
• Participate in patient discharge processes through "Start of Care Assessments" and "Re-evaluations"
• Collaborate with operations and business development teams
• Internal case management and nursing talent development
• Ensure adherence to all relevant policies and procedures
• Maintain quality assurance of clinical documentation
• Coordinate care with other healthcare providers

Qualifications of the RN Case Manager:

• Valid RN license in the state of practice
• At least 2 years of recent hands-on RN nursing experience
• Ability to travel within the designated territory
• Reliable transportation, valid driver’s license, and pass MVR check
• Current CPR certification from AHA
• Willingness to supervise in-home skilled nursing care for diverse patient populations
