Job summary

UnitedHealth Group—OptumCare is seeking a Clinic Case Manager in Las Vegas, NV to support patients with high-risk conditions and navigate the healthcare system. You’ll collaborate with physicians, nurses, pharmacists, and social workers to empower patients and families for a better quality of life.

Responsibilities include performing assessments, coordinating transitions of care, and improving outcomes while complying with standards.

Qualifications

• Valid NV RN License.
• Current BLS Certification or willingness to obtain within 3 months of hire.
• 2+ years of job-related experience in an acute care setting.
• Knowledge of medical terminology; nursing assessment and follow up on patient's needs.
• Proven ability to work with MS Office and EMR systems.

Responsibilities

• Role embedded within the primary care clinic, working directly with patients and care teams to provide transitional case management.
• Perform assessment/screening of health conditions and collaborate to address needs and goals.
• Pursue interventions to reduce risk of condition exacerbation, ER and hospitalization utilization.
• Support transition of care from ED or inpatient to outpatient settings; assist with referrals and appointments.
• Aim to achieve Quality Measures outcomes by reducing Gaps in Care (HEDIS).
