Job summary

CVS Health is seeking a Registered Nurse for case management to coordinate care for catastrophically and chronically ill members across the continuum of care. You will engage with members in person or by phone, develop comprehensive care plans, and collaborate with providers, employers, and insurers to optimize health outcomes.

Requirements include an active and unrestricted RN license in the state of residence and 3+ years of clinical experience, with 2+ years in case management or home health.

Qualifications

• Active and unrestricted Compact Registered Nurse (RN) licensure in the state of residence.
• 3+ years clinical practical experience with preference in diabetes, CHF, CKD, post-acute, hospice/palliative, cardiac, home health with Medicare members.
• 2+ years case management, discharge planning and/or home health care coordination experience.

Responsibilities

• Coordinate case management activities for catastrophic and chronically ill members across the continuum of care.
• Interact with members telephonically or in person; may visit home, worksite, or physician’s office.
• Develop care plans to optimize wellness, outcomes, and return to baseline or work.
