Job summary

CVS Health is seeking a full-time Case Manager to coordinate all case management activities, evaluating medical needs and promoting member wellness. The role requires active licensure (RN, SW, LCSW, LPC, LMFT), strong communication, and travel within a 125-mile radius for a 1-day field assignment.

Applicants should have case management experience, excellent telephonic skills, and be highly organized, with flexible hours including two evenings per week. A CCM certification is preferred.

Qualifications

• Current/Active Clinical Licensure (RN, SW, LCSW, LPC, LMFT).
• Case Management experience.
• Strong customer service orientation and problem solving.
• Excellent communication/telephonic skills.
• Excellent motivational interviewing skills/ability to build rapport telephonically.
• Excellence in documentation and compliance with regulatory standards.
• Highly organized, self-driven with no direct supervision.
• Ability to travel within 125 miles round trip for 1 day in the field.
• Variable work schedule with 2 days weekly until 9pm local time.

Responsibilities

• Coordinate all case management activities with members to evaluate medical needs and facilitate wellness.
• Develops strategy to address issues to outcomes and opportunities to enhance wellness through integration.
• Instructs programs and procedures in compliance with network management and clinical coverage policies.
