Job summary

CVS Health is seeking a Community Care Case Manager to coordinate assessment, planning, facilitation, care coordination, evaluation and advocacy for patients and families with complex health needs.

You will liaise with healthcare providers, employers, and insurers; develop care plans; facilitate integrative care; and ensure documentation and regulatory compliance in a dynamic environment.

Qualifications

• Associates Degree or Nursing Diploma (REQUIRED).
• Bachelor's Degree preferred.
• Active and unrestricted Compact Registered Nurse (RN) licensure in OH or state of residence.

Responsibilities

• Liaise with member/family, employer, providers, insurers and healthcare personnel as appropriate.
• Coordinate case management activities for complex or chronic cases across care continuum.
• Interact with members telephonically or in person; may visit homes, worksites or physician offices.
• Assess medical and vocational status; develop and implement a care plan to optimize wellness and outcomes.
