The role requires a BSN or higher and an active RN license, plus 1+ year of case management experience. You will assess patients, develop care plans, monitor progress, and maintain confidential records to ensure high-quality, coordinated care.

Qualifications

• BSN or higher from an accredited program.
• Active and unencumbered RN license.
• 1+ years of case management experience preferred.
• Strong written and verbal communication to coordinate with care team.

Responsabilités

• Conduct comprehensive patient assessments including medical history and needs.
• Collaborate with providers, patients, and families to plan care.
• Monitor progress and adjust care plans as needed, communicating changes.
• Serve as main contact between patient and providers for coordination.
• Maintain up-to-date records in accordance with confidentiality standards.
