Job summary
Heart to Heart Hospice is seeking an RN Case Manager to provide comprehensive care coordination for patients facing serious illness in Sugar Land, TX. You will perform initial and ongoing assessments, identify needs, and develop individual care plans.
In this full-time role, you will collaborate with physicians, social workers, and other clinicians, attend IDG meetings, and participate in on-call coverage. A valid RN license and a driver's license are required, along with reliable
Qualifications
- Graduate of accredited Nursing program (Diploma/ADN/BSN).
- Current state RN license.
- Valid driver''s license and reliable transportation.
- Two years RN experience preferred in healthcare.
Responsibilities
- Assess patients' physical, psychosocial and environmental needs and ADLs.
- Develop and revise patient care plans (POC) as necessary.
- Coordinate care with patients, families, physicians and healthcare professionals.
- Participate in Inter-Disciplinary Group (IDG) meetings and care planning.
- Provide on-call duties for patients and families as assigned.
- Ensure timely documentation of care activities.
- Ensure compliance with relevant laws, regulations and policies.
