Job summary

Heart to Heart Hospice is seeking a dedicated RN Case Manager in Texas to assess and coordinate care for patients with terminal illnesses. You will collaborate with doctors, family caregivers, and the interdisciplinary team to develop and adjust individualized care plans.

Responsibilities include on-call coverage, timely documentation, and ensuring compliance with Medicare regulations. A valid RN license, driver's license, and reliable transportation are required.

Qualifications

• Two years of RN experience preferred in a healthcare/clinical setting.
• Current State RN license.
• Current Driver's License with reliable transportation and ongoing auto liability insurance.

Responsibilities

• Conduct initial and ongoing comprehensive assessments of the impact of terminal disease on patients’ physical, psychosocial, and environmental needs and ADLs.
• Implement the individualized plan of care and revise as necessary.
• Collaborate with patient, caregiver, physicians and healthcare professionals.
• Attend IDG meetings and coordinate patient care planning with prioritization of needs.
• Provide on-call coverage and timely documentation.
• Ensure compliance with local, state, and federal laws and Medicare regulations.
