Job summary

Oncology Voice Network LLC seeks an experienced Registered Nurse for hospital and ambulatory case management in Downey, CA. You will collaborate with physicians, coordinate services, monitor plans, and advocate for patients to ensure timely, cost-effective care. CA license, BLS, and two years of RN experience are required.

You will work with a multidisciplinary team to coordinate access to health services, arrange transportation, and support patient self-management within regulatory guidelines.

Qualifications

• Minimum two years RN clinical experience in acute or ambulatory care.
• CA RN license and BLS certification.
• Experience in utilization review and case management.
• Strong communication and problem-solving skills.
• Proficient computer and EMR literacy.

Responsibilities

• Evaluates and identifies members needs and interfaces with PCPs and specialists to develop case management plans.
• Monitors and evaluates effectiveness of case management plans and adjusts as needed.
• Coordinates interdisciplinary care, including discharge planning and authorizations for outside services.
• Acts as a patient advocate and educator to empower self-management.
• Develops patient/family education plans focused on self-care.
