Deborah Heart and Lung Center is seeking an RN Case Manager to coordinate care across the continuum for identified patients. You will work with physicians and the care team to develop plans of care and ensure quality and cost outcomes, arranging services as needed.

Requires an NJ RN license and 3+ years acute care experience. Strong interpersonal skills and familiarity with ICD-9 coding are essential; Bachelor's degree preferred. This role offers comprehensive benefits and growth opportunities.

Formación

• Active NJ RN license; Bachelor's degree preferred.
• Three or more years of acute care nursing experience.
• Experience with case management systems or utilization review preferred.
• Excellent interpersonal skills and strong computer familiarity; familiarity with ICD-9 coding.

Responsabilidades

• Coordinate care across the continuum for identified patient groups.
• Develop and implement plans of care with physicians and health care team.
• Facilitate achievement of quality, clinical, and cost outcomes; arrange services and resources.
