Job summary

Advocate Medical Center is seeking a Case Manager who uses the nursing process to develop, implement, and evaluate outcomes for defined patient populations. You will advocate for patients, coordinate care across disciplines, and communicate effectively to ensure continuity of services and optimal results.

The ideal candidate is a Registered Nurse with a minimum of five years of experience in the target population.

Qualifications

• Graduate of an accredited School of Registered Nurse; BSN preferred.
• MSN encouraged.
• Minimum of five years of experience with the defined patient population.
• Participation in case management and utilization review encouraged.
• Current RN license in the applicable state.
• Case Management certification encouraged.

Responsibilities

• Utilizes the nursing process to develop, implement, and evaluate case management outcomes.
• Educates, consults, and assists in managing the identified patient population.
• Collaborates with care team to ensure continuity of care and cost-effective services.
• Available by pager, digital phone, and e-mail to facilitate timely outcomes.
