Job summary

Boston Medical Center in Boston is seeking an RN Case Manager to join the 4W department. The role requires assessing, planning, implementing, monitoring, and evaluating care to promote quality and cost-effective outcomes across the patient care continuum.

You will coordinate with providers, discharge planning, utilization review and community resources to ensure timely, value-driven care. Schedule is 40 hours weekly, with days / 4x10 hour shifts and every 6th weekend.

Qualifications

• ASN or BSN nursing degree from an accredited program.
• Massachusetts RN license is required.
• CCM or related certification encouraged within 24 months from hire date.

Responsibilities

• Assess patient health needs and plan care across the continuum.
• Coordinate services with providers and facilities for timely care.
• Identify appropriate resources to promote cost-effective outcomes.
• Support care management processes within the department.
