Boston Medical Center is seeking a Care Manager to assess, plan, and implement services for individuals' health needs. The position requires a strong background in nursing, including a diploma or degree in Nursing and relevant experience in medical-surgical practices.
The Care Manager will work collaboratively to promote quality outcomes while ensuring cost-effective use of resources.

Utilizing a collaborative process, the care manager will assess, plan, implement, monitor, and evaluate the options and services required to meet an individual’s health needs, using communication and available resources to promote quality, cost-effective outcomes. The care manager helps identify appropriate providers and facilities throughout the continuum of services while ensuring that available resources are being used in a timely and cost-effective manner to obtain optimum value for both the patient and the reimbursement source. This role supports the overall care management process and the department and is dedicated to handling increased volume.
Licensed to practice professional nursing as a registered nurse in the Commonwealth of Massachusetts.
Basic Life Support (BLS) certification.
Range: $40.95–$94.59 per hour. Base pay is determined by a comprehensive assessment of education, experience, licensure, and certifications.
