Incredible Health is hiring a Nurse Case Manager (RN) in Bartlett, TN. Responsibilities include conducting comprehensive patient assessments, collaborating with providers, and coordinating care plans to ensure optimal patient outcomes.

The role requires an active RN license, a BSN or higher, and 1+ year of case management experience, with proficiency in EHR systems. Shifts available include day shift; full-time, part-time, and per diem options.

Qualifications

• BSN or higher from an accredited program.
• Active RN license in good standing.
• 1+ year experience in case management within healthcare/hospital setting.
• Exceptional interpersonal and communication skills (written and verbal).
• Proficiency in electronic health records (EHR) software.

Responsabilités

• Conduct comprehensive patient assessments (history, medications, treatment plans, psychosocial needs).
• Collaborate with providers, patients and families to develop and implement care plans.
• Monitor progress and adjust care plans; communicate changes to the team.
• Serve as main contact between patient and providers for seamless coordination.
• Maintain up-to-date records and case notes, ensuring confidentiality and compliance.
