Job summary

HCF is seeking a Registered Nurse Case Manager to coordinate skilled care and communicate with the interdisciplinary team, residents and families. This role ensures adherence to physician orders and promotes quality outcomes at our Coudersport facility.

Responsibilities include pre-admission assessments, admission orders, MDS coordination, QA involvement, and ongoing discharge planning. Requirements: RN license, accredited nursing education; CPR recommended.

Qualifications

• Graduation from an accredited school of Nursing.
• Current license as a Registered Nurse with the State Board of Nursing.
• CPR certification recommended.

Responsibilities

• Pre Admission: conducts on-site assessments, verifies payer source, completes pre admission screening form, pharmacy and med surge quotes.
• Admission: review and initiate orders, communicate with care community personnel, greet resident and family.
• Coordinate residents’ MDSs and Care Conferences.
• Participates in the QA process and the 'Nurse On-call' program.
• On-going communication regarding resident goals, progress and discharge planning.
• Assumes responsibility for maintaining own professional competence.
• Oversees skilled residents care and works closely with the Office Manager.
