Memorial Hospital Belleville and Memorial Hospital Shiloh are seeking a Health Care Case Manager to coordinate, negotiate, procure, and manage the care of patients to achieve quality, cost-effective outcomes. You will work with physicians and interdisciplinary staff to resolve issues and optimize utilization management for optimal reimbursement.

Responsibilities include assessing each patient's plan of care, establishing a personalized discharge plan, and ensuring accurate clinical documentation

Qualifications

• RN licensure required.
• Bachelor's Degree in Nursing preferred.
