This role emphasizes patient and family education, collaboration, and adherence to care standards. Required RN license and BLS, with ACMA certification required or to be obtained within 2 years of hire.

Qualifications

• RN license required
• Basic Life Support certification is required.
• ACMA Accredited Case Management Certification required or must obtain within 2 years of hire.

Responsibilities

• Consults with nursing staff and multidisciplinary team to evaluate patient status and care needs (admission, length of stay, transfers, discharge).
• Monitors patient and family satisfaction; addresses questions and complaints from patients, families, and payors.
• Leads discharge planning, including patient transfers and referrals to community resources; provides discharge education.
