Qualifications

- Active RN License in the state of Illinois.
- AHA CPR certification (must be current).

- Coordinate the case management process for appropriate care.
- Oversee discharge planning processes with healthcare professionals.
- Perform utilization review to assess care compliance.
- Support PHIP to enhance outcomes.
- Maintain accurate documentation of patient care.
- Educate patients and families on case management resources.
