Qualifications

• MINIMUM EDUCATION: Graduate of an accredited School of Nursing
• PREFERRED EDUCATION: Bachelors or Masters Degree in Nursing
• MINIMUM EXPERIENCE: 2 years acute hospital patient care experience; Utilization/case management
• REQUIRED CERTIFICATIONS/LICENSURE: Texas State RN license
• PREFERRED CERTIFICATIONS/LICENSURE: ACM
• REQUIRED COURSES/COMPLETIONS: CPR or equivalent

Responsibilities

• Coordinate care along a continuum to ensure appropriate level of care and efficient throughput.
• Assist in transition management, discharge planning and readmission prevention.
• Educate patients, families and caregivers on care plans and resources.
