Qualifications

• CA RN license and AHA BLS certification are required.
• Minimum of 2 years recent case management and discharge planning experience in a hospital setting.

Responsibilities

• Coordinate management of care to ensure optimal utilization of resources and compliance with external review agencies.
• Provide ongoing assessment, care planning, plan implementation and evaluation to meet patient needs.
• Advocate for patients regarding discharge planning and resource utilization to support continuity of care and cost effectiveness.
