Job summary

Mount Sinai West is seeking a RN/Case Manager to oversee care for an assigned inpatient group, determine admission appropriateness, and develop timely care plans. The role collaborates with the interdisciplinary team to reduce LOS and optimize resource utilization.

Responsibilities include evidence-based reviews, discharge planning, and coordination with payors. Required are NYS RN licensure and a nursing degree; CCM/PRI credentials are preferred.

Qualifications

• RN license required in New York State.
• Bachelor’s degree in nursing; Master’s degree preferred.
• CCM certification preferred; PRI certification to be obtained within 1 year.
• Current NYS RN license required; re-credentialing of CCM status expected.

Responsibilities

• Reviews all new admissions to identify need for utilization review, discharge planning, and/or case management.
• Performs continued stay reviews using standardized criteria to justify inpatient stay.
• Collaborates with physicians and clinicians to expedite testing, treatment and consultations.
• Documents all clinical reviews in the medical record and UR notes.
• Maintains knowledge of UR requirements for each payor and provides clinical information for concurrent reviews.
