Job summary

The Case Manager RN at University of Miami Health System coordinates the interdisciplinary plan of care from admission to discharge. Monitors care, and serves as a liaison between patient, family, healthcare personnel, and insurers.

You will develop discharge plans, identify risk factors, and collaborate with the medical team to ensure timely, medically appropriate care while promoting quality outcomes and cost effectiveness.

Qualifications

• Graduate from an accredited school of nursing with a BSN.
• Valid Florida Registered Nurse License and BLS certification required.
• Minimum 2 years of relevant experience, with at least 1 year in hospital case management.
• Strong communication skills and ability to prioritize under pressure.

Responsibilities

• Coordinates the interdisciplinary plan of care from admission to discharge.
• Develops a discharge plan with resources to meet ongoing patient needs.
• Acts as liaison between patient/family, healthcare personnel, and insurers.
• Educates patients and families on progression of care.
• Facilitates insurance authorization and ensures timely administrative processes.
• Promotes quality care and adheres to organizational policies.
