Job summary

Qualifications

• Two years of professional experience in a clinically related area.

Responsibilities

• Coordinates case management from patient entry to post-discharge, coordinating care with physicians, social workers and team members.
• Identifies patients suitable for case management intervention based on cost, complexity or frequency of admission.
• Monitors and documents quality of care to ensure goals are understood and implemented and assesses patient/family response.
• Facilitates cost-effective outcomes by determining appropriate level of care and using standards.
• Assesses funding and insurance issues with client, family and providers to optimize utilization of services.
• Negotiates with third party payers regarding benefits and eligibility.
• Plans timely discharges and sets measurable discharge planning goals.
• May perform home or site visits and complete follow-up calls.
• Works rotating schedules including nights and weekends to meet 24/7 health system needs.
