Job summary

Tenet Healthcare's MetroWest Medical Center - Consolidated in Framingham, MA is seeking an experienced RN Case Manager to coordinate care across the patient journey, ensure appropriate level of care, and promote timely throughput and safe discharge. You will work with physicians, social workers and staff, perform medical necessity screening, and document thoroughly in the Case Management system.

The role requires RN licensure and at least 2 years in acute hospital care, with preferred bachelor's

Qualifications

• Education: graduate of an accredited school of nursing.
• Experience: two years in acute hospital patient care.
• Licensure: active RN license as required by state law.

Responsibilities

• Accurate medical necessity screening and submission for Physician Advisor review
• Identifying and referring complex patients to Social Work Services
• Collaborating with physicians, office staff and ancillary departments
• Timely complete and concise documentation in Case Management system
• Identification and documentation of potentially avoidable days
• Identification and reporting over and underutilization
• Performs related duties as required
