Job summary

WellSpan Health in Chambersburg, PA seeks a Care Management Supervisor to oversee the department's activities in a team leadership model. You will guide care management, discharge planning, social services and utilization management, ensuring alignment with hospital, system and community needs.

You will partner with hospital leadership on budgets, monitor KPIs, and drive 24/7 operational stability while promoting safety, quality and outstanding service to patients and families.

Qualifications

• Bachelor's degree required; master's preferred.
• 3 years supervisory experience.
• Lean/quality improvement experience preferred.

Responsibilities

• Oversees hiring of care management staff; trains personnel, evaluates performance, schedules and recommends actions.
• Develops budgets in collaboration with hospital leadership.
• Coordinates with hospital programs and community agencies to facilitate problem solving and service coordination.
• Supports operational goals and represents care management on hospital committees; drives effective care coordination and patient progression.
• Monitors processes, identifies improvement opportunities, and leads lean daily management initiatives.
• Ensures safety, quality and compliance with policies; promotes patient and staff safety and service excellence.
